PURPOSE OF POSITION:
Axil Health is a privately owned pharmacist collective offering both independent pharmacy, consulting, and clinical services located inside established medical facilities. The Pharmacy Intern role plays a vital role in supporting pharmacists and healthcare providers in delivering exceptional patient care within a clinical environment. This position is responsible for working with the pharmacist in charge to assist with patient engagement, documentation and chart preparation, task coordination, and administrative tasks.
MAJOR RESPONSIBILITIES & ACCOUNTABILITIES:
1. Patient Engagement:
· Conduct monthly check-in calls with patients enrolled in Chronic Care Management (CCM), assessing health status, medication adherence, and addressing any concerns.
· Make daily compliance calls to patients enrolled in Remote Patient Monitoring (RPM), ensuring adherence to monitoring protocols, addressing issues promptly and analyzing proper monitoring technique and compliance to medications.
· Perform medication reconciliations with patients who have upcoming appointments or medication concerns.
· Perform post visit check ins with patient to help close any gaps in care.
2. Documentation and Chart Preparation:
· Prepare patient charts for upcoming CCM encounters, ensuring all necessary documentation is complete and up-to-date.
· Document all patient interactions, interventions, and outcomes accurately in electronic health records (EHR) system, adhering to compliance standards.
· Prepare Patient Care Plans that are compliant with CCM regulations to be delivered to the patient to assist with their disease state management.
3. Task Coordination:
· Coordinate with clinical pharmacists regarding urgent health-related needs, medication refills, and appointment requests communicated by patients.
· Temporarily pause CCM services for patients who cannot be reached until re-engagement is achieved, ensuring continuity of care.
· Assign tasks to yourself for follow-up on previous months phone calls
4. Administrative Tasks:
· Complete data entry functions accurately, including updating patient records, documenting interactions, and generating reports.
· Utilize software programs for organizing and managing patient information effectively, ensuring data confidentiality and integrity.
5. Compliance and Time Documentation:
· Document compliance with daily tasks such as patient calls, chart preparation, and administrative duties, maintaining accurate records.
· Track time spent on each task throughout the day using designated time-tracking tools or methods to ensure efficient workflow management.
6. Professional Development:
· Stay updated on best practices, guidelines, and regulations related to Chronic Care Management (CCM) and Remote Patient Monitoring (RPM).
· Participate in training sessions or educational opportunities to enhance skills and knowledge in healthcare delivery, compliance, and patient engagement.
7. Team Collaboration:
· Communicate effectively with clinical pharmacists, fellow care managers, and other healthcare team members, sharing insights and collaborating on patient care plans.
· Participate in team meetings or case conferences to discuss patient progress, share best practices, and address any challenges or barriers to care delivery.
QUALITFICATIONS AND EXPERIENCE:
· Currently enrolled in an accredited School of Pharmacy in the United States.
· Strong knowledge of pharmacy laws, regulations, and medication safety practices.
· Excellent communication and people skills.
PHYSCIAL DEMANDS/WORK ENVIRONMENT:
· Ability to use a keyboard to enter and retrieve data.
· Must have good eyesight to view computer monitor and phone.
· Have access to a laptop/computer with internet access
· Have the ability to work remotely
